How a New Jersey med spa that had tried five agencies found its real problem: follow-up, not ads.

This New Jersey medical spa had cycled through four or five marketing agencies trying to scale, and each one promised more leads. The leads weren't the problem. Some waited more than 48 hours for a reply, and there was no CRM to track what happened to them. We moved the practice into a CRM, built automated speed-to-lead text and call sequences, and changed how its offers were presented, selling the experience rather than the product. Replies now go out within 30 minutes, often within 10. Meta Ads cost per lead dropped from $35 under the previous agency to $17, and a practice that was booking almost no one from its ads now books a couple of new patients every week.

The Problem

Agency after agency, the same result.

This med spa wanted to grow, and it had tried to buy that growth several times. Four or five agencies came and went, each focused on getting more leads.

The leads were arriving. What happened next was the problem. There was no CRM and no system for working leads, so inquiries sat in inboxes and notes. Some waited more than 48 hours before anyone reached out, and by then most had moved on or booked somewhere else.

The staff wanted to follow up. They just didn't have the organization or tools to do it consistently. No agency had looked past the ads to see it.

From the Field

“It wasn't that their team didn't care. They didn't have a system. You can't fix a follow-up problem by buying more leads. You just pay to disappoint more people. Once every lead got a text and a call within minutes and nothing fell through the cracks, the same advertising started turning into appointments.”

Audit Findings

What was really holding growth back.

01

No CRM, no visibility

Leads weren't tracked in one place, so no one could see who had been contacted, who had booked, or who had been lost.

02

Slow follow-up

Some leads waited more than 48 hours for a first reply. In aesthetics, and in a market as crowded as northern New Jersey, interest cools in minutes, not days.

03

Product ads competing with billion-dollar brands

Ads led with "Botox" and "injectables", the same message every med spa runs and that the manufacturers already spend billions on. Nothing said why this practice.

The Rebuild

Fix the system, then change the story.

01

A CRM that organizes everything

We moved the practice into a CRM and organized its pipeline so every lead is tracked from first inquiry to booked appointment, and the team can see what's working and what isn't.

02

Automated speed-to-lead

Every new lead now enters an automated text and call sequence, and the team works leads from one organized pipeline. First replies now go out within 30 minutes, often within 10, instead of days later.

03

Sell the experience, not the product

The services didn't change much. How they were presented did. Instead of advertising Botox, we advertised the experience of getting Botox at this practice: the care, the setting, and the people.

What Carries Over

Lessons for med spas on their third or fourth agency.

01

Check the follow-up before you change agencies

If leads wait hours or days for a reply, no agency can fix your results. Speed to lead is often the single biggest lever a practice has.

02

Your team needs a system, not a speech

Staff rarely drop leads because they don't care. They drop them because there's no system. A CRM and automation make the right thing the easy thing.

03

Don't advertise the product. Advertise your version of it.

Every med spa sells Botox. Patients choose the place, the people, and the experience. That's what the ad should sell.

If you've tried agency after agency

Let's look past the ads.

If you've changed agencies and the results stayed the same, the problem is probably somewhere between the lead and the appointment. Book a strategy call and we'll review your ads, follow-up, and offers together.

How a 15-year-old New Jersey med spa turned its old patient list into 4.5–5× return per campaign.

After 15 years in the same northern New Jersey community, this medical spa had seen much of its local market, and new-patient ads had become expensive. Our first test uncovered the opportunity: 15 people raised their hand for an offer, and every one of them was technically a former patient, most not seen in years. We worked with the practice to redefine who counts as a current patient, then ran returning-patient and new-patient offers side by side, each with its own language, without alienating either group. Every campaign reports its return on custom dashboards, and campaigns now return about 4.5 to 5 times their ad spend.

How a Columbus med spa produced a 50% increase in patient volume by isolating campaigns at the procedure level.

The med spa was running blended ad campaigns that hid which treatments were profitable and which were losing money. We isolated each procedure into its own campaign with dedicated landing pages, ad copy, and bid strategies. ROAS now ranges from 2x to 6x by procedure, letting the practice scale what works and stop spending on what doesn't.

How an 11-location med spa group sustained a 3.1× return on ad spend and grew booked consultations by 62%.

The 11-location med spa group had inconsistent results across markets, with each location running different programs and no portfolio-wide standard. We rebuilt the creative with a compliance-first approach, shifted to conversion-objective campaigns, and built treatment-specific landing pages for each market. The group now sustains 3.1x ROAS across all 11 locations with consistent booked-consultation volume.
