What Cash-Pay Telehealth Programs Charge in 2026

By Mike Funkhouser, Founder, Practice Growth Co·Prices checked ·Last updated

Most cash-pay telehealth categories have settled into recognizable price bands. On a month-to-month plan with the medication included, compounded semaglutide runs $99 to $299 a month (median $249), and compounded tirzepatide runs $129 to $399 (median $324). For branded Wegovy and Zepbound, telehealth brands almost never include the drug. They charge a care membership of $75 to $199 a month on top of the manufacturer's self-pay price. All-in testosterone programs sit between $129 and about $215 a month, and menopause hormone programs mostly between $89 and $149.

We checked these prices ourselves on November 4, 2026, reading each brand's own website. Where a site didn't show a price, or showed conflicting prices, we left it out rather than guessing. "From" and "starting at" prices usually mean the lowest dose, a first-month promotion or the longest prepay plan. So unless we say otherwise, the figures below are the regular month-to-month price with medication included, and each range shows how many brands it's based on.

Small samples (fewer than five brands) are directional, not a market average.

Compounded injections. Nine brands publish a month-to-month price for compounded semaglutide with the medication included. They range from $99 (Eden) to $299 (Willow), with a median of $249. For compounded tirzepatide, eight brands range from $129 to $399 at the lowest dose, with a median of $324, and some charge more at higher doses (up to $549). Several brands use one flat price at every dose. Others raise the price as the dose increases.

Prepaying lowers the monthly price a lot. For example, one brand's semaglutide drops from $289 month to month to $169 a month on a 12-month plan, and another's from $199 to $159. First-month promotions are common and often far below the regular price, so always compare regular monthly prices.

Some brands quote the medication and the membership separately. A "$60 a month" semaglutide price can become about $140 once a required $79 membership is added. Compare total monthly cost, not the headline.

Branded drugs. Manufacturers now sell directly at fixed self-pay prices:

Drug (manufacturer self-pay)Monthly price
Zepbound (LillyDirect)$299 (2.5 mg), $399 (5 mg), $449 for higher doses if refilled on time
Wegovy pen (NovoCare)$349 ($399 for the 7.2 mg dose)
Wegovy pill (NovoCare)$149, $199 or $299 by dose

Because the drug costs the same everywhere, telehealth brands compete on their care membership. Among seven brands with a month-to-month fee, that runs $75 to $199 a month (median $99), with lower prices for 12-month commitments ($49 to $74 a month at some brands) and first-month promotions from $0 to $39.

What this means for a clinic: the low end of the compounded market is crowded, so a new program rarely wins on price alone. For branded GLP-1s, the membership fee and what it includes (visits, follow-up, adherence support) is the only real point of comparison.

Men's health

Among brands that publish a flat, all-in monthly price, testosterone injections run $129 to about $200 (three brands) and testosterone cream $129 to about $215 (four brands). The gap comes mostly from how the program is structured:

  • Some charge one all-in price with labs included.
  • Some charge a low medication price plus a membership (for example $28 a month for medication plus a $155 membership).
  • Some offer steep discounts for 3 to 12-month commitments. One brand drops from $199.99 month to month to $99.99 a month on a 6-month plan.

Labs are the real dividing line. Some programs include initial and ongoing labs. Others charge separately, from $65 for an introductory biomarker test to $450 or more for a full initial panel.

Enclomiphene is mostly sold on long commitments. The headline $99 a month typically requires a 6 to 12-month plan, with month-to-month around $199.

Watch the calendar: telehealth prescribing of testosterone without an in-person visit is authorized through December 31, 2026 under the current DEA and HHS extension. Avoid "never see a doctor in person" claims until the final rule is out.

Women's hormones

Programs that include the medication cluster at $89 to $149 a month:

  • Estradiol cream or gel: $89 to $149
  • Patches: mostly $149 month to month, with one brand at $100 a month on quarterly billing

Estriol face cream sells for exactly $150 per three-month supply at both brands that publish it, which makes $150 the reference price.

Other models: some programs charge membership only, from $35 a month, and fill prescriptions through insurance or a pharmacy. Others are visit-based, at about $250 for the first visit and $150 for follow-ups.

Longevity and peptides

NAD+ injections. Five brands range from $129 to $199 a month for the medication on a monthly plan, median $150. Multi-month plans can bring it lower (one brand works out to about $75 a month on a 2-month plan).

Sermorelin. Four brands: $126 to about $200, median $164. Some sell it as a dissolvable tablet rather than an injection.

Glutathione. Three brands: $69 (on a subscription) to $99.

Membership fees are common here and change the real cost a lot. Separate fees run $25 to $155 a month, so a $129 NAD+ price can become $200 or more.

Several large telehealth brands have announced peptide programs but hadn't published prices when we checked. Compounding rules for peptides are still shifting, so confirm each product's status with your pharmacy and counsel before advertising it.

At-home lab tests

One-time lab test prices from consumer lab companies:

TestBrandsLowMedianHigh
Single-marker testosterone4$69$74$89
Men's hormone panel (multi-marker)5$149$179$259
Women's hormone or menopause panel4$129$139$149
Thyroid panel (2+ markers)4$89$109$149
Heart panel with ApoB2$169–$215

Annual membership testing (100+ markers across one or two draws) ran $199 to $365 a year at the two providers we checked.

Bundling a lab kit into a TRT, HRT or GLP-1 program is one of the clearest ways to stand apart from programs that charge for labs separately. Check state rules first: several lab providers don't serve New York, and some price New York and New Jersey differently.

Pricing models: what to copy and what to avoid

PatternUse it?
One flat price at every doseYes. Simplest to advertise and to compare
Low medication price plus a required membershipCareful. Show the total monthly cost clearly
3, 6 and 12-month prepay discountsYes, but show the month-to-month price too
First-month promotionsCommon. Always show the regular price next to it
Price shown only after an intake quizAvoid. Transparency is a differentiator

The FTC has already acted here. In 2025 it finalized an order against a GLP-1 telehealth company it said sold programs with undisclosed costs and membership commitments. If medication or a membership costs extra, say so clearly in the ad and on the page.

The rules that shape what you can advertise

  • Compounded GLP-1s: semaglutide and tirzepatide are off the FDA shortage list, so FDA now tightly limits compounded copies of the approved drugs. FDA has sent several waves of warning letters to telehealth sellers. Never say or imply "generic Wegovy" or "same as FDA-approved."
  • Google and Meta: on Google, programs that prescribe online generally need certification before any ads run, whether or not the ad names a drug, and drug-name keywords need approval too. On Meta, prescription drug ads need LegitScript certification plus Meta's own authorization.
  • Testosterone: the DEA telemedicine flexibilities run through December 31, 2026.

How we collected these prices

We visited each brand's own website on November 4, 2026 and recorded the price shown for each product, plan length, medication inclusion and any separate membership fee. We didn't use third-party price trackers in any range above. We excluded brands whose prices weren't visible, conflicted across their own pages, or only appeared in promotional "starting at" form, and counted only regular month-to-month prices in the medians unless noted. Prices change often. Check a brand's current site before relying on any single figure.

The bottom line

The market has already set its reference prices, so undercutting buys little. The opening is total cost and transparency: one clear monthly price, labs included where it makes sense, and no surprise membership. Most competitors miss at least one of those three.
