Plastic surgery marketing in Miami: 6× the search footprint and half the cost per lead.

Miami is one of the most contested plastic surgery auctions in the country, and the surgeon in this study was paying $160 to $200 a lead while his teaching credentials sat offline. We published that expertise in English and Spanish and rebuilt his ads on it. Twelve months later: about 130 ranking keywords instead of about 20, and about $80 a lead on the same roughly $6,000 budget.

The Miami Market

What a Miami patient compares before they pick up the phone.

A Miami patient who has settled on breast surgery, body contouring, rhinoplasty, or a facelift is not short of surgeons. This is one of the most competitive plastic surgery markets in the country; the patients come from Coral Gables, Brickell, Aventura, and Fort Lauderdale, and many arrive holding a quote from abroad. Many research in Spanish; all compare galleries, credentials, and reviews first.

The account in this study was paying $160 to $200 per Google Ads lead, roughly 30 to 35 leads a month on about $6,000. Our benchmark report treats mid-size metros differently from the largest markets, and calls Miami one of the most contested plastic surgery auctions in the country. It also calls an $80 lead here a win.

No practice here outbids its way past the auction; what competitors cannot match is verifiable expertise in both languages your patients use. Organic and AI search matter more here, because a ranking is not repriced by a competitor's bid. As a plastic surgery marketing agency, we treat the story as the asset and the ads as the accelerant.

Everyone in Miami can pay more for the click. The surgeon who wins is the one Google and the AI tools can verify, so the click lands on proof, not a brochure.

The Engagement

About 20 ranking keywords to about 130, and $160 to $200 a lead down to about $80.

Plastic Surgery · SEO + AI Search + Google Ads

How a Miami plastic surgeon's hidden expertise became 6× more search visibility and half the cost per lead.

This Miami surgeon teaches at a medical school, leads a surgical simulation program, and speaks at conferences around the world. Online, none of that showed. His Google Ads were paying $160 to $200 a lead in one of the most competitive plastic surgery markets in the country. We rebuilt his story around his real expertise and published it everywhere patients and AI tools look: his website, procedure pages, and a library of bilingual, expert-written patient guides. Over the next 12 months his Google ranking keywords grew about 6×, and his Google Ads cost per lead fell by half, to about $80, and held there: roughly 75 leads a month on the same $6,000 budget.

What Changed

Publishing the expertise the surgeon already had.

He needed no new reputation, only the one he had, written down where patients, Google, and AI tools could find it.

An expert story built from real credentials

A full-time medical school professor who speaks at international conferences on surgical safety, and none of it was on his site. We rebuilt his About page around those specifics, because here the credential is the differentiator. It is what took him from about 20 ranking keywords to about 130 and halved his cost per lead.

Bilingual procedure pages and 30+ patient guides

Procedure pages in English and Spanish, plus more than 30 bilingual guides that answer what Miami patients ask: recovery, cost, and whether surgery abroad is worth the risk, structured the way AI tools read. Our plastic surgery SEO and AI search work took his ranking keywords from about 20 to about 130.

Google Ads rebuilt on the new foundation

Once the pages made the expert case, we rebuilt the Google Ads account so each click landed on proof. Budget stayed at roughly $6,000 a month; leads went from roughly 30 to 35 a month to about 75, and cost per lead fell from $160 to $200 to about $80 and held.

Procedures

What a Miami patient needs to see, procedure by procedure.

Breast surgery

For breast surgery, what earns the booking is range, results across different body types with the surgeon's reasoning alongside, and pricing framed honestly, because a patient with a quote from abroad is comparing the total cost of a safe outcome. In both languages.

Body contouring

Body contouring patients know the procedure they mean, so the page has to show it, not a catch-all. Candidacy matters most: who this is for, who it is not, and what recovery looks like. Fort Lauderdale patients also want follow-up logistics across the county line.

Rhinoplasty

Rhinoplasty is where credentials do the most work. The patient wants evidence of technique, safety, and judgment, and this surgeon's surgical safety lectures were the missing evidence. The page needs those specifics, a gallery by kind of correction, and a plain account of recovery.

Facial surgery

Facelift patients in Coral Gables and Aventura want expertise that reads as real, natural results, and a consultation that is discreet from the first call.

The Benchmark

What the benchmark says about an $80 lead in Miami.

In The Plastic Surgery Ad Spend Report 2026, the five accounts with a clean before and after saw cost per lead fall 50 to 70% at roughly the same budget, and the same money bought about 2× the leads. The market did not get cheaper; the accounts stopped paying for the wrong clicks.

In the mid-size metros, $2,000 to $5,000 a month bought 7 to 10 surgeries, practice-reported. In the larger markets, DC went from $58 to $28 a lead and Miami, one of the most contested plastic surgery auctions in the country, from roughly $160 to $200 to about $80, both halved, and New York took fewer leads on purpose and reports twice the surgeries.

The figures, with their qualifiers:

  • Cost per lead after a rebuild: down 50 to 70% at roughly the same budget across five accounts; about 90% in one broken setup.
  • Lead volume after a rebuild: about 2× overall, up to roughly 3× in some accounts.
  • Before we took over: $58 to $216, six accounts. After a rebuild: roughly $20 to $80, directional only.

FAQs

Questions from Miami plastic surgeons.

The practice ran about $6,000 a month, near the roughly $5,000 median in our benchmark report, and after the rebuild that bought about 75 leads a month at about $80 each. The same $6,000 had been producing roughly 30 to 35. Spend it on rebuilt procedure pages, not a homepage.

Two clocks. The paid side responds once clicks land on pages that make the expert case; here the cost per lead fell by half and held. The search footprint took most of 12 months: about 20 ranking keywords in October 2025, about 130 in September 2026, with the first top-3 rankings in September.

The patient base spanned Miami-Dade and Broward: Coral Gables, Brickell, and Aventura on the Dade side, Fort Lauderdale across the county line. We target that draw area, and we build the Spanish pages as first-class content, because many patients here research in Spanish.

Not automatically. This surgeon had teaching and conference roles we could publish; a surgeon without them needs a different expert story. What transfers is the method: your own account as the baseline, verifiable claims, and pages built to convert.

This engagement had no Meta component, so no Meta numbers here. Google is where the patient who has already decided compares surgeons; Meta reaches the one still deciding, before the auction, in the language they scroll in. After that, plastic surgery Meta ads is the natural next layer.

How a DC-area female plastic surgeon cut her cost per lead in half with a brand that matched her work.

A plastic surgeon in the Washington, DC area known for refined, natural results was being marketed like any other clinic. Her brand didn't reflect her training, her aesthetic, or her premium pricing. We rebuilt her story around what makes her different, built a dedicated landing page around her, and restructured her Google Ads around it. On the same $5,000 monthly budget, cost per lead fell from $58 to $28, monthly leads more than doubled, and the practice reported higher-quality consultations.

How a New York plastic surgery practice nearly doubled its surgeries by getting fewer, better leads.

This New York City practice's previous agency ran Google Ads for one thing: as many leads as possible. It worked on paper, with around 170 leads a month at about $43 each. In the consultation room it didn't. The surgeon was spending his days with price shoppers and quote seekers, and only 20 to 30% of consults became surgeries. We rebuilt the program around motivated patients: tightly focused procedure campaigns, and landing pages that let his results do the qualifying. We accepted a higher cost per lead on purpose. Consults booked rose about 20%, the consult-to-surgery rate climbed to about 50%, and surgeries nearly doubled.
